You can create Forms that can be filled out in the Diary such as accident/incident forms, ABC or activity feedback forms for example. 

  • You can create Alerts to alert specific people when Forms have been submitted in the Diary.
  • You can view filled Forms in the Diary as well as export them to PDF using the Diary Reports feature.
  • You should be able to copy/re-create any forms you currently use in your care /health/eduction setting using the Forms feature.

    NOTE: creating a Form in a Diary template will mean this form can be shared with everyone in your service/care setting. If you do not manage more than one user you can simply create the form in an individuals diary without using the template feature.


To find the Form building feature:


a. In a Diary template that can be applied to multiple users:

If building a form in a Diary template for multiple users go to AH Panel>Manage Templates>Diary Templates> click on template you want to edit>create new Log for the form/s or add a new form to an existing Log in the Diary template


b. In a single users Diary:

Go to user's Diary>Diary settings>create new Log for the form/s or add to existing Log in the Diary


1. click the 'Add Form' button at the Botton of the page



2. Give the form a name and a description 


3. Set the Alert (Optional). You can set an alert to notify people when this form has been filled. Note doing this in the diary template and applying to multiple users will copy these settings). You can choose to alert 1. the users Buddies, 2. specific users in your circle/organisation.


4. Click 'Add question' to add your first question to the form (note you have to add at least 2 questions in order to save the form)


5. In the question builder pop up add question text and additional info (optional). In this case the answer type is 'Text' you can. change this to either 'date' or 'time' options.


You can also add possible answers (check boxes) as well as sub-answers (child options under parent option)in the form question:



Click 'Save' when you have composed your question to exit the question builder pop up. This will return you to the form builder pop up where you can add more questions before you save your form. You can re-arrange the order of questions, edit and delete questions in the form from here.
