Adding a Form to a Diary from a Template

Modified on Mon, 2 Nov at 3:13 PM

Multi Me allows you to add ready-made forms to your Diary without having to create them from scratch.

Forms may be provided by Multi Me or your organisation's Portal Administrator.

You can use these templates as they are or customise them to suit the individual.

How to Add a Form from a Template

  1. Open the Diary and select More (three dots) → Settings.

  2. Select the Diary Log (tab) where you want to add the form, or create a new Log.

  3. Scroll to the bottom and click + Add Form.

  4. In the form builder, click Copy from Template.

  5. Select the form template you want to use.

  6. If the form includes notifications, choose who should receive an alert when it is completed.

  7. Review the form and make any changes you need.

  8. Click Save to add the form to the Diary Log.

Customising Your Form

Once copied, you can edit the form's questions, answers and other settings without changing the original template.

Tip: Using form templates is particularly helpful when several people need to complete similar forms, such as incident reports, daily records or reviews.
